I went to Dr Smith after a recommendation from an orthopedist for a pain shot. I have extreme pain in my back and hip and needed the shot to get any relief. I waited 2 months for the appointment and the day before I was to go in, the Dr called and cancelled stating that he had never filled out the paperwork for the insurance company. In the end, I never got the shot because of Dr Smiths REFUSAL to "fill out a little paperwork." This man doesn't care about helping people. We will never return!
