I woke up ok, but I cant contact or make payment
I'll tell you what, I would be seriously worried if I was a prospective patient. Not in the quality of his service but I received a bill and when trying to make payment The phone number did not work, the website did not work and when trying to contact his offices, I found maybe 6-7 different office numbers to call for him but when I tried, one was for the CHOMP surgery center and they dont know him, one was for a Dr on Duty, one was for American Anasthesiologist group but they dont have my information or his, one was for a jail house hospital, one was for a place on Cass St but they dont know him. SO what I am saying is that I got a bill from a person I cannot contact whatsoever and I dont know how to make a payment. Not a company I want to do business with anymore

Dr. Tavakoli I would like to thank you for your services on 4/19/19 at USMD in Ft. Worth. It was by far the best experience that I have had. I felt that you performed above the bar with regards to my 5 hour surgery. I had no side effects after and I was very concerned going in. You are the best Anesthesiologist that I have ever had to use. Thank you I hope you get this because you are most definitely an excellent provider.
