Medical chart set up for failure.
She deserves one star simply for her inattention to details which started my medical chart with a rotten foundation. I’m certain no one told her I was born in Chicago, but I guess she really wanted to use the acronym “BAR” despite it being inaccurate. Other charting errors I can also prove are untrue, either from Nichols being a poor listener or the source, but no one asked me to confirm before including it. The method of doing it this way is a joke: It relies heavily on the provider and Nichols is fallible.
