I left the ER after an MVA and looked at my after-visit note. 80% was wrong. They did not list my current medications correctly; he did not mention my allergies to medications and adhesives. Worst of all, he failed to put that I have Epilepsy. He did not record my complaints or actual injuries. He sent me for x-rays when I said nothing was broken, but I am concerned about my head. He did not check for injuries around my body, and it was my 5-year-old son who found a wound from the clicker of the seat belt. He made zero effort to actually check me for injuries other than an U.S and unnecessary X-rays. He did not clean the wound on my hand, and I had to go to my primary doctor to have it cleaned and dressed. He put on my chart that I was a current drug user. I rarely drink, and I am sure they tested for drugs in all the blood work. The entire report is useless, and he needs to be held accountable. Incorrect patient information can be very dangerous.
