Basic with no skill, doesnt listen

I tried giving foster a chance for a little over two years. She never remembers what's already been tried. Never updates patient overview or assessment and when she does it's inaccurate and misleading. Forgets everything from prior visits. Doesn't listen at all. Quick to judge and she suggests the same medications and treatments over, and over again. This is after these medications have been already been tried and discussed multiple times. Has a very dull uncaring and condescending persona. How can she point a patient in the right direction when she cant remember or make notes of what has or hasn't worked? I really recommend seeing who else is out there. Good luck
