Avoid at all costs
I was sent to this “doctor” for workman’s comp. He must be paid by companies to deny any and all claims. It’s a joke/criminal. In his review, he accessed irrelevant medical history I had requested not be viewed and used restricted information in his report to my employer. He was unprofessional, noting in his report concerning my injury and pain with sarcastic and demeaning comments. The “examination” is a farce. He’s not a real and empathetic doctor, he’s bought and paid for by employers looking to discredit their employees claims. If your employer is having you see Jan Bax for any reason, insist on seeing someone else. Avoid at all costs.

He spends 10 minutes with a patient and then writes reviews stating that there is pain but no cause for the pain. I had no choice but to go to see him for workman's comp, which his report made my claim denied. I need surgery so I can go back to work, but can not get the surgery because of his review. I would never recommend him.

This Doctor's opinion cannot be trusted in workers comp cases. He made out a work comp form for the State for me, and when he wasn't chosen to be the surgeon he changed his opinion for the employer. I lost my workers comp case. I do not recommend him.

Interrupts patient responses, doesn't answer questions. Does a lot of work as independent reviewer for insurance companies and does not perform a thorough evaluation before submitting report. Enters ratings into reports without performing proper measurements. AVoid this doctor for Work Comp or personal injury issues.
