Incomplete test ordered
Dr. Watson ordered a CT SCAN for a Pt only for the brain to check for metastasis when he was well informed that there's a history of 2 other types of cancer - breast and ovarian cancer, and all the other MDs from the medical treatment team in the past we're ordering PET/CT scan of the almost full body - from head to thighs area. He didn't ask the oncologist directly about ordering the right test (CT scan from head to thighs), instead he asked him if he objects the CT SCAN for the brain, and of course the oncologist didn't object. But he never asked him about all the areas of concern.
