Came into the exam room without any results or even a chart, proceeded to go over options for treatment and after choosing one she called me 3 days later and said she wanted to go over options in case my insurance didn't cover it. (Without even trying) This led to a 3 week delay in treatment over the holidays along with playing phone tag just to be told things that were absolutely untrue about medications and my insurance. She was going off of "memory" about our office discussion, most physicians will write things down instead of relying on their memory.
