I never had a good experience with Dr Forde, but my last one was awful. My asthma worsened significantly because Dr Forde prescribed me a children's dose of a new medication. My new doctor was gobsmacked. Thankfully Dr Forde is no longer practicing in Prescott and won't have a chance to hurt any other patients in town like this.

Refused to fill out FMLA
For two weeks Dr office had FMLA paperwork so my wife could get time off to drive me to appointments and he would not fill them out after many calls and visits to the office. We missed the deadline to submit because of him.

A friend's visit(s) Numbness and pain in both legs since August 2022 Was dismissed by Forde, stating "that's what alcohol does." Friend is having femoral bypass surgery today in right leg due to 99.9% blockage Outrageous!

I am a new patient to his office. He was very polite and treated me with the respect that a patient should receive. He did not rush, and listened to ALL of my issues. The office was very clean and the entire staff was pleasant.
