Impossible to contact or reach this doctor. I have a business card from him, a patient agreement from him, and paperwork from Guardian Angels Rehab that all list 3 wrong numbers for him. All of these resources are different. None of them reach his office but are in actuality Health Partners. Even Google has the wrong phone number for him. We have an elderly family member that is weak and confused and is unable to pass information on to us the family. After 2 weeks I wish for communication.
