She is Tier 1 but does not follow up with prior authorizations. This is after the script is sent to pharamcy. My husband called our insurance company to see if script is covered. It is covered but we can not get script because her office will not call not fax info to insurance company for the prior approval needed. The office should have never called in script to the pharmacy if they do not fax prior approval. Does not need to be Tier 1
