my insurance company made him my PCP and after i authorized my transfer of med records he had his office call me 3 weeks later to say they dont want me for patient because i have 93 pages of records . no other excuse would be given to me. now his name is on my insurance card and i need a renew on my disability placard and i drove to his office gave him the p/work and he would not sign saying he did not have enough info, i have been on disabilty for 15 years now i have to pull the licence plates off my car! .VERY DISAPOINTED SHAME ON YOU!
