My sister was treated by Mr. Dautel and requested a note to confirm her visit. Over the phone with me in attendance Mr. Dautel said that he didn't have his computer when she was seen in his office therefore he would not provide a note to confirm her visit until she returned to his office some time in April. When she requested a hand written note he refused yelling at her that he wanted full payment prior to providing the note and that he would no longer treat her.

Needed a short written statement to verify my visit with him today but he refused to provide it stating that he didn't have his computer. Asked for a hand written statement but he refused to provide it stating that he wanted full payment prior to providing the statement. The insurance provider had not even been billed and yelled that her that he would not continue to see her and hung up the phone. Office was difficult to find with no name on the location and did not appear professional.
