Optometrist is listed as in network for UHC; prior to appointment I had ascertained this with his office. But at visit, I was told he is out of network for lenses prescriptions, and therefore I was over-charged (4X my copay)! Turned out he can't claim to be out of network for refraction, so billing was ultimately reversed, but not without undue time and grief with the office. I'm never going back!
