S. Lagermann, MSPT, sent a below par & unprofessionally written report containing many non factual, inaccurate, even dishonest things about me (patient) to my referring doctor. eg: I had a FALL -I had reported never had a fall. eg: Home exercise program, plans and goals have been discussed w/ patient - No such were discussed. eg: PLAN = 3 days per week of session using including aquatic equipment or ultrasound - In Reality, 1 per week session where there is no such equipment existed
