Biography: Jennifer Phlipot, MSN, APRN, FNP-C obtained her Bachelor of Science degree in nursing at Miami University in Oxford, Ohio. She earned her Masters of Science in Nursing through the Family Nurse Practitioner Program at The Ohio State University in Columbus, Ohio. Jennifer is Board Certified as a Family Nurse Practitioner and she strives to build healthy relationships with patients in order to achieve long-term wellness. Biography: Erin McKibben, MS, APRN-FNP earned her Bachelor of Arts in Psychology at the University of Dayton and her Master of Science in Nursing degrees from The Ohio State University. Prior to joining Stillwater Family Care, she served as a nurse practitioner in urgent care. She is certified in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS).

Biography: Daniel Elshoff, MD joined Premier Physician Network/Upper Valley Medical Center in 1999. A graduate of Wright State University School of Medicine with a specialty in Family Medicine, Dr. Elshoff completed his residency at Miami Valley Hospital. He is a member of the American Academy of Family Practice and the Ohio State Medical Association and has his Ohio Medical license.

Biography: Guillermo "Bill"Trevino, MD is a Board Certified Internal medicine physician. He is an avid believer in disease prevention. Biography: Heather Ray, CNP received her Master's of Science Family Nurse Practitioner from Graceland University in 2013. Ms. Rayreceived her Bachelor of Science in Nursing from the University of Phoenix in 2010.

Biography: Thomas R. Zink, DO graduated from Ohio University Heritage College of Osteopathic Medicine in Athens, OH. He completed his residency in orthopedic surgery at Doctors Hospital in Columbus, OH. He completed a shoulder and elbow fellowship at the University of Pittsburg Medical Center in Pittsburg, PA. Dr. Zink is board certified in orthopedic surgery. He is a member of several organizations including the American Osteopathic Association, American Academy of Osteopathy, National Guard Association of the United States, Ohio Osteopathic Association, American Academy of Orthopaedic Surgeons, Ohio Orthopaedic Society, and the American Medical Association. Dr. Zink is a Captain in the Ohio Army National Guard. He is the team physician for Troy City Schools. He is the Upper Valley Medical Center orthopedic department chair and Upper Valley Medical Center orthopedic trauma liaison. Dr. Zink has a special interest in knee arthroscopy and replacement, and shoulder arthroscopy, anatomic, reverse, revision, and replacement surgery.

Biography: Joyce Tebbe, MSN, APRN, ACNP-C, attended Wright State University where she studied to become an acute care nurse practitioner. She has worked in various capacities over the past 25 years both as a nurse practitioner in primary care as well as the specialty settings of cardiology and nephrology, and as a registered nurse in a cardiac catheterization lab and intensive care unit. She has also participated in clinical research involving new medications and medical devices. Ms. Tebbe is a long-time resident of Covington where she and her husband have raised three children. They enjoy spending their spare time with friends, family and grandchildren.

Biography: Andrew Diller, MD is a native of the Dayton area and completed his undergraduate and medical school education at Wright State University. After finishing residency training in family medicine at Good Samaritan Hospital (closed in 2018) in Dayton, he completed an additional year of fellowship training in geriatrics at Miami Valley Hospital.

Biography: Kathryn Lorenz, MD is a board certified family practice physician who earned her medical degree from the Boonshoft School of Medicine, Wright State University, Dayton, OH. She completed a family practice residency at Dayton Community Family Medicine, Dayton, OH. Dr. Lorenz joined Premier Physician Network in 2009. Dr. Lorenz received many honors during her training: Introduction to Clinical Medicine Award, Montgomery County Medical Society Alliance Scholarship, and the Ranville Scholarship. She was also a finalist for Outstanding First Year Medical Student.

She brings seven years of advanced care nursing experience to her career as a family nurse practitioner.

Biography: Roxanne Robbins, APRN received her Master of Science in Nursing, Family Nurse Practitioner, from the University of Cincinnati in 2019. Roxanne received her Bachelor of Science in Nursing from Western Governors University in Salt Lake City, UT in 2016 and her Associate of Science in Nursing from Edison State Community College in Piqua, OH in 2013. Roxanne is certified as a Family Nurse Practitioner by the American Academy of Nurses Practitioners. She has been a registered nurse since 2013. She brought six years of experience as an intensive care nurse to her career as a Family Nurse Practitioner.

Biography: Leah R. Gale, APRN, CNP, FNP earned her Bachelor of Science in Nursing at Ohio University and Master of Science in Nursing and Family Nurse Practitioner from Indiana Wesleyan University.

Biography: Chelsea Warner, CNP, earned her Master of Science in Nursing from University of Cincinnati in 2016. She received a Bachelor of Science in Nursing from University of Mount St. Joseph in 2011. She is board certified as an Adult-Gerontology Primary Care Nurse Practitioner through American Nurses Credentialing Center. Chelsea has been a registered nurse since 2011. She brings 7 years nursing experience with a background in neurology, progressive care, trauma and emergency medicine.

Biography: Pradip Vyas, MD is a graduate of SMT NHL Municipal Medical College in Gujarat, India. He completed his internal medicine residency at Interfaith Medical Center in Brooklyn, NY. Dr. Vyas has been a primary care physician for more than 35 years.
