Biography: Elizabeth R. Swartzwelder-Cozad, MD graduated from Wright State University Boonshoft School of Medicine. She completed her family medicine residency at Wright State University. Biography: Joseph P. Allen, MD, FAAFP is board-certified in family medicine and is a graduate of Wright State University School of Medicine. He completed his residency through the Dayton Community Family Practice Residency Program at Good Samaritan Hospital (closed in 2018). Dr. Allen's special interests include pediatric health, sports medicine and preventive medicine. Dr. Allen is also the Premier Physician Network Regional Medical Director. Kayla is certified as a Family Nurse Practitioner from the American Academy of Nurse Practitioners. She has been a registered nurse since 2017. She brings four years of emergency and trauma nursing care to her career as a family nurse practitioner.

Biography: Anoopa Hodges, DO has shared her expertise in managing acute and chronic illness in the adult population, including geriatrics, since 2004. Dr. Hodges has focused on preventative care medicine in a personal and friendly atmosphere since 2009. As a doctor of osteopathic medicine, she is able to perform manipulative treatment as well. As a Primary Care Physician, she oversees all aspects of a patient's care and refers to specialists when necessary. She is Board Certified by the American Board of Internal Medicine.

Biography: Ronald Pohlman, MD is a graduate of the University of Dayton and The Ohio State University College of Medicine. He completed his residency in Family Practice at St. Elizabeth Medical Center in Dayton. He is certified in Family Practice by the American Board of Family Physicians, and in Hospice and Palliative Medicine by the Academy of Hospice and Palliative Medicine. Biography: Isaac Cade, CNP, earned his Master of Science in Nursing in Family Nurse Practitioner from Ohio University in 2014. He received a Bachelor of Science from Ohio University in 2010. He received his Associate of Science in Nursing from Ohio University in 2009. He is board certified as a Family Nurse Practitioner by the American Academy of Nurse Practitioners. Isaac has been a registered nurse since 2009. He brings nursing experience from his background in medical-surgical intensive care and in-patient psychiatry to his career as a family nurse practitioner.

Biography: Samantha M. Loel, APRN-FNP earned her Master of Science, Family Nurse Practitioner, and Bachelor of Science in Nursing at Wright State University. She is a member of the Ohio Association of Advanced Practice Nurses and the American Association of Nurse Practitioners.
