Biography: Elizabeth R. Swartzwelder-Cozad, MD graduated from Wright State University Boonshoft School of Medicine. She completed her family medicine residency at Wright State University. Dr. Swartzwelder-Cozad is a member of the American Academy of Family Physicians, Ohio Academy of Family Physicians, and the American Medical Association. She is board certified in family medicine.

Biography: Julio Soto, MD, FAAFP is a family medicine physician who earned his medical degree from Ross University School of Medicine. He completed his clinical fellowship in urology at Emory University School of Medicine and his categorical in family medicine at the University of Toledo College of Medicine. Dr. Soto is a member of a number of professional societies including: Continental Who's Who, American Health Counsel, American Academy of Family Medicine, American Medical Group Association, Ohio Academy of Family Medicine, International Society of Men's Health, Sexual Medicine Society of North America, and the American Urological Association. Dr. Soto is board certified by the American Board of Family Practice.

Biography: Robert M. Kohut, MD graduated from University of Cincinnati College of Medicine. He completed residency at University Hospitals Case Medical Center. Dr. Kohut is board certified by the American Board of Urology. Dr. Kohut offers urology care services including: treatment of kidney stones; bladder, kidney, and prostate cancer care; lower urinary tract symptoms; urinary tract infections; incontinence; and vasectomies. Dr. Kohut received the Commendation Medal for Service from the US Air Force in 2018. Additionally, he was recognized in 2016 and 2017 with the Best Hospital Award in Air Force.

Biography: Salva Ahmed, MD is a graduate of Akron University and Wright State University School of Medicine. She completed her residency in Family Practice at Good Samaritan Hospital (closed in 2018) in Dayton. Dr. Ahmed is certified in Family Practice by the American Board of Family Physicians.

She completed her residency at Good Samaritan Hospital (closed in 2018) in Dayton. Dr. Long-Prentice is certified in Family Practice by the American Board of Family Physicians. Dr. Long-Prentice has a special interest in women's health and has been with Premier Physician Network since 1994.

Biography: Warren Ljungren, MD is a graduate of Dartmouth College and the University of Cincinnati College of Medicine. He completed his residency at St. Elizabeth Medical Center in Dayton. Biography: Joel Wehrmeyer, MD earned his medical degree from the Wright State University Boonshoft School of Medicine. He completed an internship and residency from St. Louis Children's Hospital. Dr. Wehrmeyer is certified in pediatrics and has been practicing in southwest Ohio since 2006 and joined Premier Health Pediatrics -Huber Heights in 2009.

Biography: Yangshu Linda Pan, MD is a graduate of The Ohio State University College of Medicine in Columbus, OH. She completed her gynecology and obstetrics residency at Johns Hopkins Hospital in Baltimore, MD where she served as the Equity Chief Resident.
