Would NOT recommend. He forgets seeing you as a previous patient. Forgets what injections he scheduled you for and doesn't even chart your progress correctly. Their notes are not accurate. 4 years with them and only worse over time.

Midwest Pain Clinic has been a lifesaver. My back issues are manageable and day to day living is much less painful thanks to Dr. Cook
