Biography: Dr. Jeffrey Jamesis a graduate of the University of Dayton and Ohio University College of Osteopathic Medicine. He completed his family medicine residency training at Trihealth Bethesda. Dr James completed his fellowship training in sports medicine through Trihealth Bethesda through which he also spent time with Beacon Orthopedics and sports teams from Xavier University, College of Mount St. Joseph, and Sycamore High school. Dr. James is board certified in family medicine with added qualification in sports medicine. He practices withPremier Orthopedics. Dr. James is Medical Director and team physician for University of Dayton Athletics. He is also Medical Director for the Miami Valley Hospital South sports medicine programs and is team physician for Centerville High School athletics. Dr. James' clinic practice focuses on non-operative orthopedic care for adolescents and adults, as well as the full spectrum of sports medicine needs. Dr. James has training in the use of musculoskeletal ultrasound for both diagnosis as well as use for needle and procedural guidance. Along with ultrasound, Dr. James has special interest in orthobiologic injections such as prolotherapy, platelet-rich plasma (PRP), and stem cell procedures. Biography: Elizabeth R. Swartzwelder-Cozad, MD graduated from Wright State University Boonshoft School of Medicine. She completed her family medicine residency at Wright State University. Dr. Swartzwelder-Cozad is a member of the American Academy of Family Physicians, Ohio Academy of Family Physicians, and the American Medical Association. She is board certified in family medicine.

Biography: Terez Metry, MD is a graduate of Zagazig University in Egypt. She completed her Internship in Internal Medicine at St. Mary's in Rochester, New York and her residency at Miami Valley Hospital in Dayton, Ohio. Dr. Metry is certified in Internal Medicine by the American Board of Internal Medicine. She has special interest in Women's Health and Diabetes. Dr. Metry has been associated with Premier Physician Network since 1996.

Biography: Anessa Alappatt, MD is a graduate of Northeastern Ohio Universities College of Medicine. She completed her residency in family medicine at Indiana University's Methodist Hospital. Dr. Alappatt is board certified by the American Board of Family Medicine. She has been with Fairborn Medical Center and Premier Physician Network since January 2008.

Biography: Angela Long-Prentice, MD is a graduate of the University of Dayton and Wright State University School of Medicine. She completed her residency at Good Samaritan Hospital (closed in 2018) in Dayton. Dr. Long-Prentice is certified in Family Practice by the American Board of Family Physicians. Dr. Long-Prentice has a special interest in women's health and has been with Premier Physician Network since 1994.

Biography: Christopher Lauricella, DO is a graduate of Florida State University and Ohio University College of Osteopathic Medicine. He completed his residency in Family Practice at Grandview Medical Center in Dayton. Dr. Lauricella is certified in Family Practice by the American College of Osteopathic Family Physicians. Biography: Anjana H. Shah, MD is a graduate of Kasturba Medical College in India and completed her residency at State University of New York (SUNY). Dr. Shah is certified in Internal Medicine by the American Board of Internal Medicine and has been with Premier Physician Network since 1993. Dr. Shah has special interest in diabetic patient management and patients with other chronic diseases.

Biography: Anoopa Hodges, DO has shared her expertise in managing acute and chronic illness in the adult population, including geriatrics, since 2004. Dr. Hodges has focused on preventative care medicine in a personal and friendly atmosphere since 2009. As a doctor of osteopathic medicine, she is able to perform manipulative treatment as well. As a Primary Care Physician, she oversees all aspects of a patient's care and refers to specialists when necessary. She is Board Certified by the American Board of Internal Medicine.

Biography: Julie Larson, MD is a graduate of Carroll College and the University of Washington School of Medicine. She completed her residency in family medicine at the University of Utah at Salt Lake City. Dr. Larson is certified in family practice and geriatrics by the American Board of Family Physicians. She has a special interest in women's health and geriatrics. Dr. Larson has been with Premier Health Family Care -North since 2002.
