Biography: John Terry, MD is a graduate of the University of Kansas School of Medicine. He completed an internship in internal medicine at the University of Missouri at Kansas City and a residency in neurology at University of Texas Southwestern Medical Center. Dr. Terry completed fellowships in neurocritical care at Johns Hopkins Hospital Medical Center and interventional neuroradiology at the University of Iowa. Biography: Terez Metry, MD is a graduate of Zagazig University in Egypt. She completed her Internship in Internal Medicine at St. Mary's in Rochester, New York and her residency at Miami Valley Hospital in Dayton, Ohio. Dr. Metry is certified in Internal Medicine by the American Board of Internal Medicine. She has special interest in Women's Health and Diabetes. Dr. Metry has been associated with Premier Physician Network since 1996.

Biography: Elizabeth R. Swartzwelder-Cozad, MD graduated from Wright State University Boonshoft School of Medicine. She completed her family medicine residency at Wright State University. Dr. Swartzwelder-Cozad is a member of the American Academy of Family Physicians, Ohio Academy of Family Physicians, and the American Medical Association. She is board certified in family medicine.

Biography: Anessa Alappatt, MD is a graduate of Northeastern Ohio Universities College of Medicine. She completed her residency in family medicine at Indiana University's Methodist Hospital. He completed his residency in Family Practice at Grandview Medical Center in Dayton. Dr. Lauricella is certified in Family Practice by the American College of Osteopathic Family Physicians. Dr. Lauricella has a special interest in cardiology and sports medicine. He has been with Premier Physician Network since November 2006.

Biography: Kelly earned her Master of Physician Assistant Studies from Kettering College of Medical Arts in Kettering, OH in 2018. She completed her Bachelor of Science in Biology from The Ohio State University in Columbus, OH 2015. She received her Associate of Applied Science in Nursing from Sinclair Community College in Dayton, OH in 2012. Rebecca is certified as a family nurse practitioner by the American Academy of Nurse Practitioners. Rebecca has been a registered nurse since 2012. She brought four years of experience as a cardiac nurse to her career as a family nurse practitioner.

Biography: Anjana H. Shah, MD is a graduate of Kasturba Medical College in India and completed her residency at State University of New York (SUNY). Dr. Shah is certified in Internal Medicine by the American Board of Internal Medicine and has been with Premier Physician Network since 1993. Biography: Anoopa Hodges, DO has shared her expertise in managing acute and chronic illness in the adult population, including geriatrics, since 2004. Dr. Hodges has focused on preventative care medicine in a personal and friendly atmosphere since 2009. As a doctor of osteopathic medicine, she is able to perform manipulative treatment as well. As a Primary Care Physician, she oversees all aspects of a patient's care and refers to specialists when necessary. She is Board Certified by the American Board of Internal Medicine.

Biography: Charles W. Hall, MD, PhD graduated from the University of Cincinnati College of Medicine in 2000. He completed his residency in Neurology at the Mayo Clinic. Dr. Hall completed a fellowship in EEG/Epilepsy at the Mayo Clinic. Dr. Hall received his PhD in Molecular and Cellular Physiology from the University of Cincinnati. He is board-certified in neurology.
