He then returned to Washington University for residency training in Neurological Surgery, which included a seven-month experience as Registrar in Neurological Surgery at Beaumont Hospital in Dublin, Ireland. He then remained at Washington University for an Endovascular Surgical Neuroradiology fellowship at the renowned Mallinckrodt Institute of Radiology. He maintains a focus in cerebrovascular neurosurgery, with numerous research projects and publications in leading journals. Thanks to Dr. Arias' training as both a neurosurgeon and endovascular neurroradiologist, he can provide both perspectives in a single sitting. This allos a comprehensive, unbiased perspective, offering both open surgical and minimally invasive endovascular approaches to cerebrovascular lesions, such as aneurysms, arteriovenous malformations, moyamoya disease, stroke, and carotid stenosis. Dr. Falci is a graduate of Georgetown University in Washington, D.C. and completed his neurosurgical residency at Georgetown in 1992. He received his undergraduate training at Princeton University, Princeton, N.J. Dr. Falci initiated and designed the First Transplantation of Embryonic Nerve Cells for Spinal Cord Injury Project at Craig Center for Spinal Cord Research in collaboration with the Spinalis SCI Research Unit, Karolinska Institute, Huddinge University Hospital, Stockholm, and the department of Neurosurgery, University of Uppsala in Sweden. (J. of Neurotrauma, Vol. 14, No. 11, November, 1997). His specialties include: Surgical treatments for posttraumatic spinal cord tethering, postraumatic syringomyelia, posttraumatic neuropathic pain, spinal deformities, spasticity, as well as research in neuropathic pain and stem cell development for spinal cord injury.

The possibility of brain surgery is a frightening concept; I consider it my privilege and responsibility not only to offer my clinical and technical expertise, but to guide my patients through possibly the most difficult time in their lives. I tailor treatment to my patients' goals and values, and strive to deliver it with a collaborative spirit not only putting my patients and their families at the center of the process, but combining compassionate care with the latest techniques to deliver personalized and comprehensive treatments.

Dr. Hughes thinks informed, shared-decision making with patients is crucial in a surgical practice. For some patients, surgery may not be the best option or an option at all, and Dr. Hughes believes knowing when not to operate is just as important as knowing when to operate. When surgery is an option, surgical indications, goals, risks and benefits should be clear, as well as if there is a low, intermediate, or high probability that an operation will improve a patient's problem and fulfill his or her expectations.
